To become a provider with Freedom Health, you must be credentialed with our Health Plan. We have selected Aperture to provide primary source verification in Credentialing providers to become part of our Network. Physicians previously credentialed (within the last 2 years) with Aperture can go on-line to www.APP-ONE.com, update the application, and print. Forgot your password? Simply call Aperture at 800-398-0335. Review all the existing information to make sure the data is complete and up to date.

Complete the following:

• Mandatory Questionnaire
• Attestation/Consent Release Form

Attach the following documents:

• All state professional licenses
• Current Curriculum Vitae or Resume
• Copy of the DEA and/or CDS (if applicable)
• Copy of professional liability & workers’ compensation declaration page
• ECFMG certificate (if applicable)
• W-9 form
  

Additionally, Primary Care and OB-Gyn providers require an on-site visit. Once we verify all your credentialing information, we'll present it to the Health Plan Credentialing Committee. This committee is composed of community physicians for peer review, and the Health Plan Medical Director. If you're accepted for participation in our network, you'll receive a "welcome" letter within 60 days of the date of the decision, or sooner, if required by state law.
